#1 The items Bolek’s Craft Supplys sells are for ADULT use or ADULT SUPERVISED use only. Many items in our catalog contain sharp edges, small parts, hot glue, candy oil, fragrance oils, essential oils, plastic bags, etc...these can be dangerous to young children. PLEASE BE CAREFUL!
#2 In the contiguous USA Bolek’s Craft Supplys Inc. Pays Freight on orders over $100.00. Orders under $100.00 will have a flat rate of $10.00 for shipping. We reserve the right to ship orders the cheapest way. Orders will ship via U.S. MAIL, Parcel Post or Priority, or Fed Ex. Customers are responsible for any extra charges for shipping preferences if the cost is more than the cheapest. All other orders pay actual shipping cost & must specify carrier. U.S. Funds ONLY!!
#3 When placing phone orders, please have your credit card and order ready with item #’s before you call.
#4 All Next day air, Second day air, priority, express, and special handling deliveries, will be charged actual shipping charges, and will ship within 24 hours from time order is received, Monday-Friday ONLY!
We reserve the right to refuse special handling orders in our peak season.
#5 When ordering please print Name, Shipping Address, Mailing Address, City, State, Zip Code & Phone number starting with Area Code. All credit card orders must state correct billing address for verification.
#6 Give Quantity, Item Number, Price of Item & Total Cost if ordering more than one of that item.
#7 Ohio State Residents Only must add 6.75% Ohio State Sales Tax (or must have on file with us a blanket certificate of exemption, and also must write your vendors number on every order that we receive).
#8 Send payment with order. Make checks payable to Bolek’s Craft Supplys. We accept Master Card,
Visa, American Express & Discover Card charges by Mail, Fax, Phone & E-Mail.

#9 WE DO NOT ACCEPT C.O.D ORDERS! WE DO NOT HAVE OPEN CREDIT ACCOUNTS!!!
#10 All returned checks will be charged a $25.00 fee. No exceptions.
#11 If there is a problem with your order after you receive it, please write or phone 1-800-743-2723 within 14 days after you receive it. DO NOT RETURN ANY MERCHANDISE UNLESS YOU OBTAIN AUTHORIZATION!
We will correct our mistakes at our expense.
#12 Exchanges and Returns: Merchandise may be exchanged or returned within 30 days of receipt of order at the consumers expense. Exchanged merchandise will be charged the actual shipping or $10.00 (whichever is greater) to return merchandise back to you. No returns or Exchanges on essential oils, carrier oils, lotions, butters, creams, fragrance oils, soaps, waxes, seasonal, closeouts, opened, special ordered, or damaged merchandise.
Refunds: Merchandise may be sent back for a refund. Bolek’s Craft Supplys Inc. reserves the right to deduct shipping costs accrued to ship original order and up to 25% restocking fee may apply. No returns on seasonal, closeouts, opened, special ordered, or damaged merchandise.
#13 Orders should ship within 3 days of time received, except in peak season. Please allow 2 weeks for delivery on the WEST COAST. 10 days for delivery on the EAST COAST!!!
Please Note: UPS charges $11.00 for any incorrect delivery address. Please make sure your address is correct or this charge will be added to your invoice.
